What to Bring to Your First Dental Visit
You can download a medical history form from our website and complete it at your convenience.
Along with this form, bring the following to your first dental visit:
- Your identification card or driver's license
- Your insurance card
- Your copay or financial obligation
- Any recent or relevant dental records or films
- A list of questions you'd like to ask
What to Expect at Your First Dental Visit
Expect a warm greeting on your first visit (and every visit after). At this appointment, you'll meet the dentist and our team members, including your hygienist.
Typically, your first visit includes a dental checkup and teeth cleaning. We may take x-rays if you do not have relevant or newer films on hand.
After your appointment, we set aside time to answer your questions and address your concerns.
